It's that time of year again!  Please log into your patient portal and update your demographic information.  If you are unable to login, we will ask you to complete a form at your first appointment of the year.  You can also upload copies of your ID cards, or bring them to the office.

Most insurance companies encourage annual Preventive Visits, and will usually waive copays and deductibles.  Some even offer incentives like gift cards for completing screenings like mammograms and colonoscopies.
